MA-TF 1 Application Procedure

1. Review the Position Description(s) for the position(s) you are interested in as well as the General MA-TF1 Membership Requirements.


2. Complete an application packet, which includes the following:

   • Responder Information Sheet
   • Transmittal Sheet
   • Activation Reimbursement

   • Policy/Procedure (sign)
   • Vendor Contract
   • FEMA Candidate Medical Screen Form
     (requires that you have a physical exam with your PCP)
   • MA-TF1 CORI Request Form
   
   
   • Submit a copy of your drivers license (and DOT card if applicable)
   • Submit a copy of your passport (if you currently have one)
   • Submit copies of any professional licenses/Certifications (MD, RN, Engineer, EMT-P, EMT-B, CPR, First Aid, Hydralics, Hoisting, etc.)
   
   
   • Official documentation of immunizations against:
   • Tetanus (Td), Measles (MMR), Hepatitis-A, Hepatitis-B and Polio
   
   
   • Documentation of Training in: (courses available on-line)
   • Emergency Response to Terrorism: Basic Concepts
   • IS-100, IS-200, IS-700, IS-800
   • VFIS OSHA Respiratory Protection Standard Questionnaire
