MA-TF 1 Application Procedure

1. Review and agree to the General MA-TF1 Membership Requirements.
2. Review and agree to the FEMA General Requirements.
3. Review the Position Description for the position you are applying for (click title to review):
   • Canine Search Specialist
   • Communications Specialist
   • Hazardous Materials Specialist
   • Heavy Equipment and Rigging Specialist
   • Logistics Specialist
   • Medical Team Manager: (Physicians with Emergency Department experience ATLS, ACLS, PALS preferred)
   • Medical Speciallist
   • Rescue Specialist

   • Technical Information Specialist
   • CDL class "A" drivers with experience
     
4. Complete a full application packet, which includes the following:

2. Resume
3. Memorandum of Agreement (MOU)
4. OF 612 Application for Federal Employment
5. Appointment Affidavit
6. Responder Information Sheet
7. Transmittal Sheet
8. Activation Reimbursement
9. Commitment Declaration
10. Beneficiary Form
11. Policy/Procedure (sign)
12. Vendor Contract
13. FEMA Candidate Medical Screen Form : Requires that you have a physical exam with your Primary Care Physician
14. MA-TF1 CORI Request Form
15. Submit a copy of your drivers license (and DOT card if applicable)
16. Submit a copy of your passport (if you currently have one)
17. Submit copies of any professional licenses/Certifications (MD, RN, Engineer, EMT-P, EMT-B, CPR, First Aid, Hydralics, Hoisting, etc.)
18. Form I-9, Employment Verification
19. Official documentation of immunizations against:
    • Tetanus (Td), Measles (MMR), Hepatitis-A, Hepatitis-B and Polio
20. Documentation of Training in: (courses available on-line)

• IS-100, IS-200, IS-700, IS-800
• VFIS OSHA Respiratory Protection Standard Questionnaire
• WMD Terrorism Awareness for Emergency Responders (search for "AWR-160-W WMD" if the link doesn't work)
